Step 1: Download and print the Dealer Application. Step 2: Complete and Sign the Dealer Application. Step 3: Fax the Dealer Application to: (610) 617-0307. You will be emailed your temporary Dealer number Step 4: In order to obtain your permanent authorized dealer number, you MUST remit the original signed Dealer Application and Agreement to the following address: Adobe Acrobat Reader is required To view and print the Dealer Application. Upon approval a copy of the executed agreement will be mailed to you for your records. If you require an original executed agreement for your records, please print, complete, sign and mail two complete original Dealer applications. ASK will execute both originals and mail one original back to you. This Authorized Dealer Agreement will be executed and renewed on an annual basis. All terms are based on credit card, advanced payment via check or wire transfer or COD. If you would like to establish credit terms, download and complete the ASK Credit Application.
